Liability Release Waiver ALBERTA PERFORMING ARTS ACADEMY CAMP LIABILITY RELEASE AND WAIVER *

Acknowledgment of Risk I, the undersigned parent/guardian, acknowledge that my child’s participation in the Alberta Performing Arts Academy Camp (the “Camp”) involves activities such as dance, drama, music, and other arts-related or recreational activities. I am aware that these activities may involve certain risks, including but not limited to minor injuries (e.g., sprains, strains) or more serious injuries. I voluntarily agree to allow my child to participate in the Camp, fully aware of the potential risks involved. Assumption of Responsibility I assume full responsibility for any risks or injuries, whether known or unknown, arising out of my child’s participation in the Camp activities. I understand that the Alberta Performing Arts Academy, its owners, directors, employees, volunteers, instructors, and agents (collectively, the “Released Parties”) are not responsible for any injury, loss, or damage that my child may sustain while participating in any Camp activities. Release of Liability In consideration of my child being allowed to participate in the Camp, I hereby waive, release, and forever discharge the Released Parties from any and all claims, suits, actions, or causes of action for damages of any kind arising out of or relating to my child’s participation in the Camp. This includes, without limitation, claims arising from negligence of the Released Parties. Medical Consent In the event my child requires emergency medical treatment and I cannot be reached, I consent to the Camp personnel seeking and administering any necessary medical care. I agree that I am responsible for any costs associated with such medical treatment. Compliance with Rules I understand that my child is required to follow all rules and instructions provided by the Camp staff. I agree that if my child fails to comply, the Camp may remove my child from participation without refund. Acknowledgment of Understanding I have read this Liability Release and Waiver, fully understand its terms, and understand that I am giving up substantial rights by signing it. I sign it freely and voluntarily, without any inducement or coercion.
